Required Job Specific Competencies: o The Clinical Coordinator of CCC Rehabilitation Services must be able to demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s / resident’s / resident’s status and interpret each patient’s / resident’s requirements relative to age specific needs and to the care needed for effective treatment. Must be able to provide administrative guidance and consultation appropriate to the age level of development of the Associate. o Must demonstrate the skills necessary to evaluate and treat referred patients / residents / residents within their scope of practice. o Must be able to coordinate all MDS flows, minutes management and daily therapy schedule including the weekend schedule in order to ensure that the standard of care is met [to meet the established RUG level]. o Ensures that all CCC Rehab staff use the transitional documentation tools effectively and have been trained in consistent manner. o Completes the electronic board and electronic schedule and completes 6N reports weekly showing all indicators of care delivered. o Orients all new CCC staff and establishes an orientation schedule for new employees. o Ensure the transition to 6N is timely, efficient and meets all Medicare standards for all disciplines. o Coordinates a monthly meeting to discuss updates/changes and any ongoing issues to be resolved. o Works with Leader of Coding and Compliance to ensure monthly rehab documentation audits are completed and feedback is given to each staff member. o Ensures that all staff are timely with documentation and that all charges are submitted each night. o The Clinical Coordinator must be competent in the clinical application of therapeutic modalities within the scope of their licensure and be able to assess the clinical work of other therapies under his/her direction. Ensures that all ACP modalities are ordered for each patient/resident and track the monthly usage by therapist through 6N. o Mature, responsible, accountable, energetic and creative individual with excellent verbal and written communication skills. Demonstrates success in managing the daily operations, budgetary and personnel responsibilities. Demonstrates ability to deal with others in a professional, effective and courteous manner. Exhibits leadership skills through mentoring & development of staff. o Consistently displays a high standard of medical and business ethics. Complexity of Duties: o Diversified duties require the Clinical Coordinator to apply the skills necessary to evaluate and treat referred patients / residents in a manner consistent with treatment plan. The treatment plan of patients / residents reflects the necessary application of therapeutic intervention and application of appropriate modalities as indicated through initial evaluation. o The Clinical Coordinator is responsible for the yearly completion of assigned Associates performance evaluations and competencies, orientation of new Associates along with daily supervision and coordination of respective clinical area.
